Q. What are the evaluation fees?
A. The cost of an assessment is a flat fee. This fee varies depending on the type of evaluation required. The fee is set during the initial intake phone call, prior to the start of the assessment. Evaluation fees include the intake, school observation (if appropriate), consultation with the school and other providers, testing, up to two feedback meetings (i.e., one with the parents, and one with the child and parents), and a written report. Additional meetings, including attendance at school meetings, are billed separately. One half of the evaluation fee is due at the time of the initial parent meeting, and the second half is due upon the initial testing session. 

Q. Do you take insurance?
A. Dr. Jacobs is not paneled with any insurance companies, but will provide you with a superbill to present to your PPO insurance carrier for out-of-network reimbursement, upon request. It is recommended that you speak with your insurance carrier to determine what is covered, and the specific requirements. 

Q. What forms of payment do you accept?
A. Cash, checks, Venmo, and Zelle payments are accepted. 

Q. What should I tell my child about why he/she is coming for testing?
A. The answer to this largely depends on the age, developmental level, and functioning of the child. In general, it is advised that parents inform their children that they will be doing some activities that will help us get to know them better. Some activities will be like schoolwork, some will be like puzzles, and some will involve drawing or creative work. Dr. Jacobs is happy to provide more specific guidance to parents during the intake meeting after having gained more information about the individual child.
