Child History Form:
Prior to the first meeting, please complete the Child History Form. This form requires information about your child's early development and school history. It may be preferable to complete the chid history form from home so that can access documents that may help you recall important milestones, i.e. report cards, photo albums, baby books, etc. Please print and complete the child history form and bring it to the intake appointment.
Release of Information Form:
If Dr. Joanna Jacobs is evaluating, or has evaluated, your child and you would like her to speak with another provider, please email with your request. In addition, please fill out and sign the Release of Information form acknowledging that Dr. Jacobs has your permission to discuss results and recommendations with the designated person(s).
